Signing Passport Applications
Please be aware that GPs are no longer able to counter sign Passport Forms following new guidance from the Passport Office below:
The counter signatory must not:
- Be related by birth or marriage
- Be living at the same address as the applicant/parent of applicant
Additional Update: “In the past it was ok to use your Doctor/GP to countersign your application. Due to high demand and complaints received from medical professionals you are now not able to use your registered doctor as a counter signatory (however if you have a friend or colleague who is a doctor this is still permitted).”

Copy of Vaccination Record
If you require a copy of your vaccination record then please download, complete and submit the Private Work Request Form.
The form and documents can be;
- posted to ‘Imperial College Health Centre, 40 Princes Gardens, London, SW7 1LY’
- scanned and emailed to [email protected]
Once received we will endeavour to contact you within ten working days to inform you of an estimated completion date.
Private Letter/Sick Note
If you require a private letter or sick note then please download, complete and submit the Private Work Request Form.
The form and any related documents can be;
- posted to ‘Imperial College Health Centre, 40 Princes Gardens, London, SW7 1LY’
- scanned and emailed to [email protected]
Once received we will endeavour to contact you within ten working days to inform you of an estimated completion date. We will not begin processing any requests until payment has been received.

For any other requests please download, complete and submit the Private Work Request Form.

Making Payment
Please do not make any payments until you have received a quote from the Health Centre.
