Medical Record on Online Services
Imperial College Health Centre offers patients the ability to view their Medical record via Online Access or through the NHS App.
Please follow the guidance on how to apply on the online access page. Please note if you have Online Access already but have not yet got access to your medical record then please complete the registration via the NHS App.
