Requesting a copy of your Medical Record
Your medical records can be accessed electronically via the SystmOnline portal. If you would like a hard copy of your medical record then please download, complete and submit the Private Work Request Form.
The form can be;
- posted to ‘Imperial College Health Centre, 40 Princes Gardens, London, SW7 1LY’
- scanned and emailed to [email protected]
Once received we will endeavour to contact you within ten working days to inform you of an estimated completion date.
