Change of Address

It is important that our records are kept up-to-date in case we need to contact you. If you do not inform us of a change in your contact details, we may be obliged to take you off our patient list.

We do not have access to any information held about you by Imperial College/The Royal College of Music.

All information, including address and contact information, given to us is treated in the strictest confidence and will not be shared with any third party (including College) without your consent.

We often contact our patients by telephone or e-mail so please be sure to include these where possible.

You can do this in a number of ways:

This is the quickest & most efficient way of updating your contact details.


You also use this service to make and cancel routine appointments on line.

Please include your:

  • Current forename and surname in full
  • Date of birth
  • Current (new) address
  • Previous address (where you moved from)
  • Mobile telephone, land telephone, IC internal telephone and email address (where possible)
We cannot process your request without all this information.

Please note this email address is for administrative matters.  Any medical questions should be addressed to the clinicians preferably during routine consultations.  For urgent issues/questions, please phone reception who will help you.

Please remember that to be registered with us you need to be in our catchment area.

 

If you no longer live in our catchment area, please read the following:

We are unable to register you as an NHS patient as you do not live within our catchment area.

Students at IC and RCM:

You must now register with a local GP and your NHS medical records will be returned to the PCT, however you can be seen here during our opening hours as an "out of area patient". Each time you visit us you will be asked to complete a short NHS form which allows us to send a record of any treatment you receive here, to be added to your NHS notes. Your NHS medical record will not be held here and you will not be able to use our out of hours emergency services.

Staff at IC & RCM:

You must now register with a local GP - Your NHS medical records will be returned to the PCT. You cannot be treated here anymore except for work place accidents / emergencies and travel vaccinations

Local Resident:

You must now register with a local GP - Your NHS medical records will be returned to the PCT. We are no longer able to treat you at this surgery.

To Find A Local GP:

You can call NHS Direct on 0845 46 47, or visit their website at www.nhs.uk, to find a list of GP's surgeries in your new local area.

Alternatively you can contact your Local Primary Health Care Trust (you will find your local one listed either on the NHS website or in the telephone directory) to find out details of local surgeries or visit a local library.
